athenahealth is a cloud-based EHR and practice-management platform widely used by ambulatory practices and groups.
athenahealth is one of the largest cloud, network-based ambulatory platforms, strong with independent practices and multi-specialty groups. Because it is API-forward and Caesar Health is an official marketplace partner, the agents plug into athenahealth’s scheduling, billing, and clinical data cleanly and write every result back.
Does Caesar write back into athenahealth, or does my staff re-key it?

A finished encounter maps to four narrative sections and fourteen structured clinical domains in athenaOne.
Narrative sections: history of present illness, review of systems, physical exam, and assessment and plan. Each is written to its own athenaOne section, not concatenated into one box.
Structured domains: diagnoses, orders, medications, allergies, vitals, problems, vaccines, patient goals, billing services, visit reasons, and medical, surgical, social and family history.
Two details matter more than the list:
- It writes into your note templates, not generic ones. Caesar reads your practice's athenaOne templates and keeps the ones your clinicians already use, then checks the write landed where it was meant to.
- Diagnoses are resolved to SNOMED and verified before they are written. Wrong coded data is worse than none.
Scheduling runs against athenaOne's live availability: slot search, booking, rescheduling, cancellation, confirmation and per-patient appointment lookup. The phone agent quotes real openings.
Insurance is read from the coverage on file. When Caesar adds a payer to a patient, it matches against athenaOne's insurance package catalog instead of free-texting a payer name.
Nothing files silently. Anything the agent is unsure of becomes a task with the call recording, the transcript and the extracted data attached.
How the athenahealth EMR integration API works
Auth. OAuth2 client credentials. Caesar exchanges its credentials at /oauth2/v1/token for a bearer token scoped to athena/service/Athenanet.MDP.*, then works against your practice ID. Tokens are cached and shared, so one agent turn doesn't trigger a handshake per call.
Rate limits. athenahealth rate-limits hard, including on the token endpoint. Caesar backs off and retries on its own, with a shared quota across the process. A phone agent that stalls mid-sentence because the EMR throttled a lookup is worse than no agent. This is the part of an integration a demo never shows, and it decides whether it survives a Monday morning.

The wider surface. athenahealth publishes more than 800 API endpoints in two families: a proprietary API with write access across administrative, clinical and financial functions, and FHIR R4 APIs. Its FHIR server is at api.platform.athenahealth.com/fhir/r4, with a preview environment for testing before production.
Access. Production integration runs through the athenahealth Marketplace. Caesar Health is listed there. Credentials are issued against your practice and are yours to revoke.

athenahealth ships Ambient Notes. Caesar works around the visit.
Ambient Notes is athenahealth's ambient documentation feature inside athenaOne. athenahealth launched it in October 2024 with two third-party ambient models, Suki AI and iScribe AI, and said clinicians can switch between models and that more would be added over time.
So the EHR vendor made ambient scribes interchangeable options inside its own product. A practice on athenaOne has a scribe choice already.
Caesar's surface is the work around the visit:

Yes, into structured fields, not as an attachment. Four narrative sections and fourteen structured clinical domains are written into your athenaOne note templates, with diagnoses resolved to SNOMED and the write verified afterwards. A clinician still reviews and signs.

How does Caesar connect to athenahealth?+
OAuth2 client credentials against /oauth2/v1/token, scoped to your practice ID, with automatic backoff for athenahealth's rate limits. Caesar reads and writes patients, appointments, note sections, diagnoses, orders, medications, allergies, vitals, histories, insurance and documents.
Is Caesar Health an athenahealth Marketplace partner?+

Is it HIPAA compliant?+
Yes. Caesar signs a BAA. PHI is encrypted in transit and at rest with customer-managed keys, access is role-based with tenant isolation per organization and location, and access to PHI is logged. SOC 2 Type II is in progress.
