What is an AI EHR?
An AI EHR is an electronic health record in which artificial intelligence does part of the work around the chart, such as drafting the note, filing incoming documents, answering patient calls or preparing orders and claims, instead of only storing what staff type in. An AI EMR means the same thing.
An AI-native EHR is an electronic health record designed around artificial intelligence from the start, so documentation, coding and patient communication run inside the record itself. Adopting one means moving the practice’s chart onto it.
An agentic EHR is an electronic health record, or a layer on top of one, in which AI agents finish multi-step tasks, such as booking the visit, filing the fax or queueing the refill, and record the result in the chart, rather than suggesting the next step for a person to take.
Three ways to get AI into the EHR
"AI EHR" covers three different purchases. They differ on one question: does your chart move?
- AI features inside the EHR you have. Your current vendor adds search, summaries or an ambient scribe. The record stays. The AI suggests; staff still finish the task.
- An AI-native EHR. A new record built around AI. Documentation, coding and patient messages run inside it. You migrate your chart to get it.
- An agentic layer on the EHR you keep. AI agents work in your existing EHR: they answer the phone, book into your schedule, file the fax to the chart and push the note back. The record stays. The agent finishes the task.

AI EHR vs. AI EMR
HealthIT.gov describes an electronic medical record as "a digital version of the paper charts in the clinician’s office." An electronic health record goes further: EHRs "are built to share information with other health care providers, such as laboratories and specialists" (HealthIT.gov).
In practice, and in how vendors sell AI, "AI EMR" and "AI EHR" name the same product. This page uses EHR.
What makes an EHR agentic?
An AI feature suggests. An agent finishes.
A system counts as agentic when it passes three tests:
- It completes the task end to end. The visit is booked, not proposed. The fax is filed to the right chart, not flagged.
- It writes the result into the EHR. The appointment is in the schedule. The note is in the chart. Work that stops at a dashboard is a suggestion someone still has to key in.
- It fails into a named queue. When the agent is unsure, the task lands with a person, with the reason attached. Silent failure disqualifies it.

What an agentic EHR does in a clinic day
- Phones. Answers every call, books, reschedules and cancels against live availability in the EHR. See Phone Agent.

What the rules say about AI in the EHR
Certification. The ONC Health IT Certification Program "ensures that Certified Health Information Technology meets the technological capability, functionality, and security requirements adopted by the U.S. Department of Health and Human Services" (HealthIT.gov). Certified products are listed on ONC’s Certified Health IT Product List.
AI transparency in certified EHRs. ONC’s decision support interventions criterion, § 170.315(b)(11), defines a predictive decision support intervention as "technology that supports decision-making based on algorithms or models that derive relationships from training data and then produces an output that results in prediction, classification, recommendation, evaluation, or analysis." If a developer supplies one in a (b)(11)-certified product, "the developer must include source attributes and other requirements for that Predictive DSI" (HealthIT.gov). Source attributes describe how the model was built, tested and kept current.
Medicare programs. CMS’s Promoting Interoperability program works "through the use of certified electronic health record (EHR) technology (CEHRT)" (CMS). Clinicians report it through the MIPS Promoting Interoperability performance category. Billing chronic care management requires recording the patient’s demographics, problems, medications and allergies "using a version of certified electronic health record (EHR)" (CMS, MLN909188, June 2025).
What follows for a practice that bills Medicare: keep a certified EHR as the record. AI can run beside it, or inside it, without changing that.
Replace your EHR or add agents to it?
An AI-native EHR fits when:
- you are opening a new practice and have no chart to move
- your current EHR is being retired or no longer meets your needs
- you have the time and staff for a migration and retraining
An agentic layer fits when:
- you bill Medicare and rely on a certified EHR for MIPS or CCM
- your history, templates and staff habits live in the EHR you run
- you run more than one EHR across sites
- you want one workflow working this month, not a migration this year
Both are honest bets. Say out loud which one you are making before the first demo.

- Does my chart move? If so, what comes across and what stays behind?
- Is the product on ONC’s Certified Health IT Product List, and for which criteria?
- Which tasks does the AI finish, and which does it only suggest?
- Where does a task go when the AI is unsure? Who sees it?
- Can I see what the AI decided on each task, and why?
- Does the result land in my EHR, or in a separate dashboard?
- Will you sign a business associate agreement?
- For predictive features, where can I read the source attributes?

It takes over tasks: answering calls, filing documents, chasing forms. Staff keep the exceptions and the patients who need a person.
