The decision

ThoroughCare is care coordination software built by clinicians. Your care managers work in it, with AI that summarizes, transcribes calls and suggests care-plan goals. Caesar Health runs AI agents and a care-program engine alongside your EMR. The agents place outreach calls themselves, and your staff do the clinical work.

Both put AI in the program. ThoroughCare's AI helps the care manager. Caesar's agents take part of the care manager's work.

"No published claim" means we found nothing on ThoroughCare's own pages on 22 October 2026. It does not mean the product lacks it. Ask ThoroughCare directly.

The structural difference

ThoroughCare's About page describes an assistant for the care manager: "Use our AI assistant to help engage patients, document notes, and automate next steps." The care manager places the call, inside the platform, and the AI transcribes it and suggests what comes next.

Caesar's Phone Agent places the enrollment and reminder calls itself, on the cadence you set, and writes the outcome to the patient's record. Your staff take the calls that need a clinician. Their minutes are captured as they talk.

AI time is not clinical-staff time under Medicare's care-management codes. Caesar never counts it. The agent takes the work that was never billable.

Minutes across programs

A patient in CCM and RPM generates minutes that could count toward either. ThoroughCare tracks time logs and shows billable time per program. We found no published claim on how it splits one call's minutes across two programs.

Caesar counts each minute once and proposes the split that pays best under the rules, across RPM, RTM, CCM, PCM, APCM, BHI and Collaborative Care. A staff member confirms before the time counts toward a claim. Pinned choices are never moved.

Where ThoroughCare is strong

  • Behavioral health. Dedicated CoCM and BHI pages, with PHQ-9 and GAD-7 screeners and automatic CPT selection.
  • AI that helps the care manager. Summaries, call transcription, conversation guides and AI-suggested care-plan goals.
  • Device breadth. It lists BodyTrace, Dexcom, SMARTMETER, Tenovi and Withings, plus Validic, and orders devices for patients.
  • Annual wellness visits. An AWV workflow sits in the same platform.
  • Clinical advisory. Its clinical team helps design programs and workflows.

Choose ThoroughCare if

You have care managers who own the calls and want AI to make each of them faster. You need AWV in the same platform, or a wide device catalog.

Choose Caesar if

Your program stalls on reach and paperwork, not on the length of each call. You want agents to place the routine calls, one phone for front desk and care team, minutes split across programs automatically, and a billing gate a person releases.

Running both is possible. Pick one system to count minutes, so no minute is counted twice.

What to ask on either demo

  • Log one call for a patient in CCM and RPM. Where do the minutes land, and who confirms the split?
  • Which calls happen with no staff member on the line?
  • Show the care plan going back to your EHR. Does the note, the time or the charge go too?
  • Trigger a reading outside a threshold. Can the threshold be set for this one patient?
  • Hold one patient at billing. What evidence is shown, and who releases it?
  • Who submits the claim, and from which system?

Make both teams prove it.

Bring one real workflow with patient details removed. Follow it from the first request to the final record.

  1. Log one staff call for a patient enrolled in two programs.
  2. Show where those minutes are counted and who confirms them.
  3. Trigger a reading outside a threshold and follow the alert.
  4. Hold one patient at the billing gate and read the evidence.
