Caesar Health vs ChronicCareIQ

ChronicCareIQ organizes the care-management work your in-house team already does. Caesar's agents take on part of that work. Compare who makes the calls, which phone they use and how minutes reach the billing queue.

How each one works.

The decision

ChronicCareIQ is care management software for in-house care teams. It organizes the outreach, documentation, time tracking and reimbursement work your staff already do. Caesar Health runs AI agents and a program engine alongside your EMR. The agents take on part of the monthly work, and your staff do the clinical part.

Both count care-management time and allocate it to programs. The difference is who makes the calls, and on which phone.

"No published claim" means we found nothing on ChronicCareIQ's own pages on 22 October 2026. It does not mean the product lacks it. Ask ChronicCareIQ directly.

The structural difference

ChronicCareIQ describes its job as documenting work your team already does. Its About page says it works by "automatically documenting all non-face-to-face activities that your team is already doing." The care manager still makes every call. The software captures the minutes and sorts them into programs.

Caesar moves part of the work off the care manager. The Phone Agent places enrollment and reminder calls on the cadence you set and writes the outcome to the patient's record. Staff take the calls that need a clinician. Their minutes are captured as they talk and allocated across the patient's programs.

AI time is not clinical-staff time under Medicare's care-management codes. Caesar never counts it. The agent takes the work that was never billable: reaching patients, chasing readings, scheduling, paperwork.

Phone: layer versus line

CallerIQ connects to the phone system you already run. The CallerIQ page says it supports documentation "alongside your existing phone system, not replace it." The integrations page shows logos for RingCentral, Spruce, Dialpad, GoTo and others.

Caesar Phone is a phone. The staff phone is a softphone with extensions, ring groups, voicemail, transfers and the patient's chart on screen when a known patient calls. See the medical phone system. Practices that keep their current phone can forward calls to the Phone Agent instead.

If you are happy with your phone system, a capture layer is the smaller change. If you are paying for a phone system and a care-management tool separately, ask whether one system can carry both.

Where ChronicCareIQ is strong

  • Program breadth. TCM and chronic pain management sit on its program list beside CCM, RPM and BHI/CoCM.
  • Named EHR connections. It has a page per EHR, including Tebra and eClinicalWorks, with what it reads and writes for each.
  • Phone-system neutrality. It works with the phone you have.
  • Published security attestations. It states SOC 2 Type II for three consecutive years. Caesar's SOC 2 Type II is in progress.
  • Risk scoring. RiskIQ ranks patients by risk in a color-coded dashboard.

Choose ChronicCareIQ if

You have a care team that owns the monthly calls, a phone system you intend to keep, and you run TCM or chronic pain management programs. You want software that captures and sorts the work, not agents that do part of it.

Choose Caesar if

Your program stalls on reach: patients not called, consent not captured, readings not chased. You want agents to run that outreach, one phone for front desk and care team, and a billing gate a person releases. You run RPM, CCM, PCM, APCM, BHI or Collaborative Care, or you are moving to APCM.

Running both is possible. Pick one system to count minutes, so no minute is counted twice.

What to ask on either demo

  • Log one call for a patient enrolled in two programs. Where do the minutes land, and who confirms them?
  • Show the call being placed. Which phone made it, and who dialed?
  • For your EHR, what writes back: the note, the time, the charge or an audit log?
  • Trigger a reading outside a threshold. Who is alerted, and how fast?
  • Hold one patient at billing. What evidence is shown, and who releases it?
  • Which outreach, if any, happens without a staff member on the line?

Make both teams prove it.

Bring one real workflow with patient details removed. Follow it from the first request to the final record.

  1. Log one staff call for a patient enrolled in two programs.
  2. Show where those minutes are counted and who confirms them.
  3. Trigger a reading outside a threshold and follow the alert.
  4. Hold one patient at the billing gate and read the evidence.

Frequently asked questions

Is Caesar Health a ChronicCareIQ alternative?

Yes. Caesar runs RPM, RTM, CCM, PCM, APCM, BHI and Collaborative Care with agents doing the outreach and staff doing the clinical work. ChronicCareIQ also lists TCM and chronic pain management.

Does ChronicCareIQ have a dialer?

ChronicCareIQ describes CallerIQ as a layer that integrates with 50+ phone systems. The CallerIQ page says it works "alongside your existing phone system, not replace it." We found no published claim of a built-in dialer. Ask ChronicCareIQ.

How much does ChronicCareIQ cost?

ChronicCareIQ does not publish a price. Its /pricing address returned a 404 on 22 October 2026. Ask for a quote based on your programs and panel.

Does ChronicCareIQ submit claims?

Its FAQ says its billing information "may be imported into your practice management/billing system." BillingIQ calculates code combinations and supports your billing team. Caesar never submits claims either. A person sets Ready for Billing and your billing team submits.

Can AI time count toward CCM or RPM minutes?

No. Medicare's care-management codes pay for clinical staff or practitioner time. Caesar counts only staff minutes. See CCM, PCM, APCM, RPM and RTM codes.

How does Caesar compare with other CCM vendors?
