Spruce calls itself “the all-in-one platform for HIPAA-compliant healthcare communication: phone, text, fax, video, and secure patient and team messaging.”
Spruce prices per user, per month, in two tiers, with a 14-day free trial and a Business Associate Agreement included for eligible organizations.
The decision
Spruce is one app for phone, text, fax, video and secure messaging. Caesar Health is an agent that answers the call and does the work in your EHR.
They are different purchases. Spruce gives your team a place to handle patient communication. Caesar's agent handles it, then leaves the result on the chart.
Many practices run both. This page says when that makes sense and when it doesn't.
Where the chart comes in
Spruce publishes real chart connections. As read on 9 October 2026:
- athenahealth. Spruce says you can sync "demographic information and the content of your Spruce conversations to the patient chart in Athena." Texts, voicemails and call logs arrive as Phone Messages. Sync is selective or automatic, and automatic sync creates a new Phone Message every 24 hours. Spruce says the integration "currently does not support fax forwarding." It needs the Communicator plan.
- ModMed. Spruce says the integration syncs "patient contacts and their demographic details only." Conversations are not written to the ModMed chart.
- Any other EHR. Spruce offers conversation links and formatted copy and paste, in a web browser.
Those are useful. A person on your team decides what gets synced, or it posts as a daily summary.
Caesar's agent works the other way round. On a call or a text, it finds the patient, searches your live open slots, and books, reschedules or cancels in the EHR. Faxes are read, classified and filed to the patient's chart. Filing to athenahealth is live today.

Where Spruce is strong, plainly
It is a complete small-practice communication tool. Phone, text, fax, video and a team inbox in one app, with iOS, Android, Mac, Windows and web apps.
The pricing is simple to read. Spruce prices per user and says it does not charge individually for texts, video calls, voice minutes or feature use. Taxes, a carrier registration fee for outbound texting and desk-phone hardware are extra, and Spruce says so.
The BAA comes with the account. Spruce says every eligible organization receives one at signup, free trial included.
The athenahealth and Elation integrations write conversation content to the chart. If that is the gap you are closing and your team will review what syncs, it does the job.
Choose Spruce if…
You are a small or independent practice and want one app for phone, text, fax and video. You want per-user pricing and a free trial. You are on athenahealth or Elation and want selected texts, voicemails and call logs synced to the chart by your team.
Choose Caesar Health if…
The painful part is what staff do after the call. You want the call answered, the patient found and the appointment booked in your EHR without a person touching it. You run ModMed or EZDERM, where booking is live. You get faxes that need to be read and filed to the right chart.

- Call the line and book a visit. What is on the patient's chart that nobody typed?
- Send a fax for a patient. Where does it land, and who filed it?
- On the athenahealth integration: what posts, when, and who chooses?
- What does the practice do at 9 p.m. when no one answers?
Frequently asked questions
Is Caesar Health an alternative to Spruce?
For the calls, texts and faxes, yes. Caesar's agent answers calls, books in the EHR and files faxes to the chart. Spruce also provides video, team chat and a patient app. Caesar's staff phone, with a softphone, extensions and ring groups, is in pilot.
Does Spruce have an AI receptionist?
Spruce's pages describe AI voicemail transcription, AI summaries and AI-generated greetings. They don't describe an AI that answers calls and books appointments. Ask Spruce what is available today.
Does Spruce sync calls and texts to the patient chart?
For athenahealth and Elation, Spruce says texts, voicemails, call logs and transcripts can sync to the chart, selectively or automatically. For ModMed, Spruce says only contacts and demographics sync. Each EHR needs the Communicator plan.
Does Spruce file faxes to the patient chart?
Spruce's athenahealth article says the integration does not support fax forwarding. Spruce says a fax is filed to a contact by hand with its "regarding" feature.
How much does Spruce cost?

Can I keep Spruce and use Caesar?
Yes. Forward the calls you want the agent to answer. Your Spruce number and your team's tools stay where they are.
Can I keep my phone number?
Yes. Forwarding needs no port. Porting to Caesar is coming with the staff phone.
