Klara and Weave

Weave (NYSE: WEAV) is a phone system with a business layer on top: VoIP phones, phone tree, call queue, forms, reviews, text to pay. On 28 May 2026 it launched an Omnichannel AI Receptionist, in early access for Dentrix, Eaglesoft and Open Dental users.

Moving a message is not doing the work

Watch what actually happens when a patient wants to move an appointment.

Klara puts the message in the right inbox and assigns it to the right person. That is a good inbox. A staff member still opens ModMed, finds the patient, checks the schedule, moves the appointment, and texts back.

Weave rings the desk. Somebody picks up. That same staff member still opens ModMed and does the same six steps.

Caesar’s agent does the six steps. It finds the patient, searches your real open slots, moves the appointment, writes it back to ModMed, and confirms with the patient. Nobody is assigned anything.

That is the whole difference. Both of those products move messages between your patients and your staff. Caesar completes the task.

The consolidation follows from it. When the work finishes in one place, you stop paying four vendors to hand the same patient back and forth.

What each of these actually is

The two are not the same kind of product, and comparing them as if they were is how practices end up buying both.

Klara is a messaging layer with a shared team inbox. ModMed acquired Klara in February 2022 and now sells it as ModMed Patient Engagement, powered by Klara. As of August 2026, klara.com redirects to ModMed’s patient engagement page. The real product is the inbox: ModMed’s page describes shared inboxes, @mentioning other staff, intelligent message routing, and messages consolidated into one patient thread.

It rides on whatever phone system you already have. Its call features are call-to-text — ModMed’s description is "Allow patients who call to start texting instead" — and voicemail transcription pulled into the thread. ModMed’s patient engagement page lists call-to-text, messaging, online scheduling, patient intake, appointment reminders, patient feedback, eStatements, and retention.

Weave is a phone system with a business layer on top. VoIP phones, phone tree, call queue, plus forms, reviews, text to pay, email marketing and fax. Plans start from $199 per month with per-plan pricing by quote.

On 28 May 2026 Weave launched an Omnichannel AI Receptionist built on Google Cloud’s Gemini Enterprise Agent Platform. It answers calls and texts and hands off to staff. Per Weave’s own page it is early access for Dentrix, Eaglesoft and Open Dental users; automatic booking and rescheduling is labelled "Dental offices only"; phone payments and multilingual support are labelled "Coming soon."

So: an inbox that does not touch your phone system, and a phone system that has just added an AI receptionist for dental practices.

Where each one is strong

Worth saying plainly, because you will find out in a demo anyway.

Klara’s shared inbox is the best part of Klara. Assignment, internal notes, @mentions, one thread per patient. If your front desk lives in that inbox, that is a real workflow and Caesar does not have one.

Weave’s phone system is a real phone system. Extensions, ring groups, call queues, voicemail boxes, handsets in the box. Caesar is not a phone system and is not trying to become one this year.

Weave has an AI receptionist now. Anyone selling you "AI that answers the phone" as a unique thing in 2026 is a year behind the market. The question is no longer whether the AI answers. It is what the AI can finish once it does.

Five things that break in this stack

1. Online booking that a person still has to accept

ModMed’s own self-scheduling walkthrough states that a practice "can either configure confirmations to be manual or automatic," and shows staff clicking Confirm Appointment to populate the calendar.

On manual, an online booking is a request in a queue. It is a message with extra steps.

Ask your vendor: which setting are we on, and who accepts the 9pm Friday request?

2. A texting number patients do not recognize

ModMed’s page describes giving patients "your textable practice number."

Ask your vendor: is the number patients text the same number they dial? If it is not, patients ignore it and staff fall back to the phone.

3. Your phone number lives at your phone vendor

Weave’s VoIP Service Terms state that the subscriber gives Weave permission to port the number to the service, that the account "must be active and in good standing in order to port out your number," and that cancelling before a port completes may release the number.

Whoever holds the number sets the pace of every change you want to make.

4. Four invoices, four support queues, no shared record

A reminder tool cannot see what the phone vendor heard. An inbox cannot see what the reminder sent.

5. Text-first automation leaves the phone ringing

Older patients call. That has not changed and no texting product fixes it.

But answering is the easy half. The half that matters is whether the thing that answered can find the patient, read your live schedule, and book — or whether it takes a message and creates a task for somebody.

What Caesar does

Caesar is an AI agent layer on top of ModMed. ModMed stays the system of record. Nothing is migrated. See how it works and the ModMed integration.

Conversational SMS that books. The SMS agent holds a real conversation and completes it against your EMR: finds the patient or creates a new one, searches your live open slots, books, reschedules or cancels, and reads and adds insurance. English and Spanish, with language detection. Not a reply template and not a handoff to a person.

A patient engagement engine you configure per clinic. Cadence, quiet hours, per-trigger enable, a clinic-level kill switch, and idempotency so a re-run cannot double-send. Live triggers include appointment reminders at 7 days, 3 days and morning-of, no-show detection and follow-up, checkout thank-you, intake, outstanding-balance reminders driven by a live EMR balance read, and dropped-call continuity — if a call drops before the patient finished, the text picks it up.

Outbound campaigns with the guardrails that matter. DNC filtering, insurance-plan exclusion, max attempts with a retry cadence, local-time windows, business-hours respect, wave sizing, and outcome classification.

A waitlist that fills itself. When a slot frees up, the optimizer promotes from the waitlist automatically. Nobody works a list.

Phone agent. Answers the line 24/7 and completes the request — books, reschedules, cancels against live EMR slots. Warm transfer to staff when a human is needed.

Fax agent. Inbound faxes read, classified, and filed to the right chart. Neither of the products on this page does this.

Patient intake. Intake documents and insurance cards read and extracted into the chart before the visit.

Ambient scribe. The visit note written back into ModMed, on the same platform as everything above.

Caesar runs on your existing number. You forward the line or assign it. Nothing is ported to us.

What Caesar does not do

Read this list before you cancel anything.

  • No phone system. No softphone, no extensions, no ring groups, no call queues, no voicemail boxes, no screen pop, no desk phones. Your phone system stays.
  • No staff inbox. No shared inbox, no assignment, no internal notes, no @mentions. If your front desk works inside Klara’s inbox, Caesar does not replace that surface today.
  • No patient payments. No text-to-pay, no card on file, no terminal.
  • No review loop. No review requests, no reputation monitoring.

Frequently asked questions

Is Klara still a separate company?+

No. ModMed acquired Klara in February 2022. It is sold as ModMed Patient Engagement, powered by Klara, and klara.com redirects to ModMed’s patient engagement page.

Is Klara a phone system?+

No. Klara is a messaging layer that runs on top of the phone system you already have. Its call features are call-to-text — letting a caller switch to texting — and voicemail transcription pulled into the patient thread.

Weave now has an AI receptionist. What is different about Caesar?+

Weave launched its Omnichannel AI Receptionist on 28 May 2026. Per Weave’s own page it is early access for Dentrix, Eaglesoft and Open Dental, automatic booking is labelled "Dental offices only," and payments and multilingual support are labelled "Coming soon." Caesar is in production on specialty medical EMRs, writing to live slots in ModMed, and the agent captures insurance and completes intake in the same conversation. Compare them on what the agent finishes, not on whether it answers.

Do I have to leave ModMed to use Caesar?+

No. ModMed stays your system of record. Caesar reads your schedule and writes appointments, documents and notes back into it. There is no data migration.

Can Caesar replace both Klara and Weave?+

Not both. Caesar replaces the layer that talks to patients and completes their requests — calls, texting, booking, reminders, campaigns, fax, intake. It does not replace a phone system, a shared staff inbox, patient payments, or a reviews product. If you are on Weave mainly for the phones and the payments, Caesar sits alongside it rather than instead of it.

What happens to our main phone number?+

It stays where it is. You forward the line to Caesar or assign it. If you are leaving a VoIP vendor, read that contract first — Weave’s VoIP terms, for example, require your account to be active and in good standing to port a number out.

Will online bookings still need a staff member to accept them?+

Not through Caesar. The agent checks live availability and writes the appointment. On ModMed self-scheduling, confirmation behaviour is a setting — ModMed’s own walkthrough describes confirmations as configurable manual or automatic. Check which one you are on.

Our patients are older and call instead of texting. Does this help?+

Yes, and the reason is not that the AI picks up. It is that the AI finishes the call — finds them in ModMed, reads the real schedule, books, and confirms. Patients who prefer text get the same thing over text.
