Longevity clinic software, compared

Longevity clinic software is the set of systems a cash-pay longevity, hormone, functional or concierge clinic runs on: a chart, booking, membership billing, labs, e-prescribing and a way for members to reach the clinic. Most clinics run one main system plus two or three add-ons. Below are seven systems and two add-ons, side by side, from each vendor's own website.

How this comparison was built

Every vendor fact below comes from the vendor's own website, read on 2 November 2026, in the vendor's own words where it matters. Prices are as published on that date.

"No published claim" means we did not find it on the vendor's pages. It doesn't mean the product can't do it. Ask the vendor.

Caesar Health makes one of the products in this list. Its row says what it runs today.

The short version

  • Healthie: one system with a patient app, memberships, programs and AI notes.
  • Practice Better: an all-in-one system with programs and courses, a public booking page and add-on e-prescribing.
  • OptiMantra: an EMR and practice management system for integrative and wellness clinics, with membership and package management built in.
  • Cerbo: a configurable EHR for functional, DPC, cash-based, IV and BHRT clinics, with subscriptions and controlled-substance e-prescribing.
  • Akute Health: an "automation-first" EHR for DPC, concierge and wellness clinics, with Hint billing as an add-on.
  • Elation Health: a primary care EHR with native membership management for direct primary care.
  • Hint: membership billing first, with a full EMR plan for direct care clinics. Some clinics run Hint's billing beside another EMR.
  • Caesar Health: the front office around your chart. It answers calls, texts, web inquiries, forms and faxes, drafts notes, and can be the chart for a clinic that has none.

"No published claim" means the capability isn't described on the vendor pages we read on 2 November 2026. Several of these vendors list third-party integrations in their marketplaces. Ask each vendor what runs today and what it costs.

Elation and voice agents. On 2 June 2026 Elation announced it had acquired Aster, whose team "built Atlas, a voice agent that automates front-office tasks for healthcare practices." Ask Elation what is available to your practice today.

Cerbo and OptiMantra. Cerbo's own About page lists its leadership team with the title "Cerbo/OptiMantra". Each product has its own website and price list.

The add-ons most cash-pay clinics run

Fullscript (supplements and labs). Fullscript calls itself a "Supplement Management & Lab Testing Platform" that "Works in your EHR". Rupa Health, the specialty lab ordering service, says "We joined Fullscript." Rupa's page says it "is free for practitioners", with patients paying "discounted wholesale prices plus a 7% service fee", as published on 2 November 2026. Healthie, Practice Better, Cerbo and others list Fullscript or Rupa integrations.

Spruce (calls, texts and secure messages). Spruce describes itself as "the all-in-one platform for HIPAA-compliant healthcare communication: phone, text, fax, video, and secure patient and team messaging." It publishes phone trees, call routing, and AI transcription and summaries for voicemails and call recordings. Plans are Basic at $24 per user a month and Communicator at $49 per user a month, as published on 2 November 2026. Akute lists a "Spruce + Hint integration".

Hint (membership billing only). Hint sells a billing-only plan that adds its "membership management and employer functionality" to the EMR you already run. Akute sells "Hint Core billing" as an add-on.

For hormone and TRT clinics

Testosterone is a Schedule III controlled substance (21 CFR 1308.13(f)). A clinic that prescribes it electronically needs e-prescribing that covers controlled substances (EPCS).

On their own pages, Healthie ("Controlled Substances E-Prescribing (EPCS)"), Practice Better ("Option to add Electronic Prescribing of Controlled Substances (EPCS)"), OptiMantra ("eRx license with EPCS") and Cerbo ("controlled substances") describe it. For the others, ask whether EPCS is included, what it costs, and how identity proofing works.

Hormone clinics also run on a lab cadence: a baseline panel, a follow-up draw, a dose review. Ask any vendor who reminds the patient about the next draw, and who rebooks the one who misses it. See hormone clinic software for the full workflow.

What none of the seven systems publishes: an AI front desk

All seven systems publish booking and patient messaging. On the pages we read on 2 November 2026, there is no published claim of an AI agent that answers the clinic's phone and books the caller. Elation's acquisition announcement is covered above.

For a cash-pay clinic, the phone is where members start. The inquiry at 8 pm, the caller asking what the program includes, the patient who needs to move a draw. A self-booking link helps the caller who goes looking for it. It doesn't answer the one who calls.

Where Caesar fits

Caesar runs the front office around your chart. You keep the system that holds your memberships, billing and prescriptions.

  • The Phone Agent answers every call, day and night. It finds the patient, books, reschedules or cancels, transfers to the number you choose when a person is needed, and leaves a summary of each call. You forward your line. No number port.
  • The SMS Agent sends reminders and handles the replies.
  • Web chat answers visitors on your site and books them.
  • Online forms collect intake before the first visit.
  • The Document Inbox reads faxes, including lab reports, and files each one to the right patient.
  • The AI Scribe drafts the visit note.

Caesar books through APIs into athenahealth, ModMed and EZDerm, works inside other web-based EMRs through a browser agent, and can be the chart for a new clinic that has none. See integrations.

You can run Caesar beside any system on this page. Your memberships, billing and prescriptions stay where they are.

Choose each one if

  • Choose Healthie if you want one system with a patient app, wearable sync, programs and AI notes, and a low entry price while you're small.
  • Choose Practice Better if programs, courses and protocols are part of what you sell, and you want deposits, packages and Rupa labs in the same login.
  • Choose OptiMantra if you run an integrative or wellness clinic that sells memberships, packages, IVs and compounded products, and you may bill some insurance.
  • Choose Cerbo if you need a configurable chart and controlled-substance e-prescribing for functional, BHRT or DPC care, and portal booking requests confirmed by your staff suit how you book.
  • Choose Akute if you run a DPC or concierge practice and want automations by patient cohort, with Hint billing and Spruce available.
  • Choose Elation if you run membership primary care, or hybrid insurance and membership, and want a primary care EHR with native membership billing.
  • Choose Hint if memberships, employer plans and recurring billing are the center of your business. Take the full platform, or the billing-only plan beside your EMR.
  • Choose Caesar if calls, inquiries, reminders, faxes and notes are where your team loses time, and you want that handled without changing the system that holds your members.

What to ask on any demo

  1. What happens to a call at 8 pm? Answered and booked, a voicemail, or a self-booking link sent later.
  2. Does a booking request become an appointment, or a request someone confirms?
  3. How do memberships bill? Recurring dates, failed-card follow-up, packages, pauses.
  4. Where do lab results land? Ordered and resulted in the chart, or a PDF someone files.
  5. Is EPCS included, and what does onboarding cost per prescriber?
  6. What do members install? An app, a portal, or nothing.
  7. What does the second clinician cost? Per clinician, per member, or per active patient.
  8. Will you sign a BAA? Members share health data with a cash-pay clinic too.

Frequently asked questions

What is the best EMR for a longevity clinic?

The one that fits how your clinic sells care. Healthie, Practice Better, OptiMantra, Cerbo, Akute, Elation and Hint each publish memberships or recurring billing, booking, labs and e-prescribing. Compare them on booking, membership billing, EPCS, the member app and price, then add a front office that answers the phone.

What software do hormone and TRT clinics use?

The same systems, with one extra check: testosterone is a Schedule III controlled substance, so the clinic needs e-prescribing that covers controlled substances. Healthie, Practice Better, OptiMantra and Cerbo describe EPCS on their own pages. Ask the others.

How much does longevity clinic software cost?

As published on 2 November 2026: Healthie from $19 a month, Practice Better from $35 a month (monthly billing), OptiMantra $99 a month for the first practitioner, Hint from $290 a month, Cerbo $281 a month per full-time prescribing provider plus $1,195 set-up, and Akute from $100 a month. Elation publishes no price. Check each vendor's pricing page before you compare.

Can I run memberships in my EMR, or do I need Hint?

Most systems on this page publish recurring billing or membership management. Hint sells a billing-only plan for clinics that want its membership and employer billing beside another EMR, and Akute sells Hint billing as an add-on.

Which longevity clinic software has an AI receptionist?

On the pages we read on 2 November 2026, the seven systems on this page have no published claim of a built-in AI phone agent. Elation announced on 2 June 2026 that it had acquired a team that built a front-office voice agent. Caesar's Phone Agent answers and books, and runs beside any of these systems.

Does Caesar replace my EMR?

No. Caesar runs the calls, texts, web inquiries, forms, faxes and notes around your chart. A new clinic with no chart can start on Caesar's.

Do I need a BAA if my clinic doesn't bill insurance?

HIPAA covers a health care provider who transmits health information electronically in connection with a covered transaction, such as an insurance claim (45 CFR 160.103), so a cash-only clinic may fall outside it. State privacy laws can still apply. Choose vendors that sign a Business Associate Agreement. See longevity clinic software for the rule.
