What an ACO gets
Your cohort, found
You set the criteria, for example type 2 diabetes with chronic kidney disease. Caesar Health returns every match across your participating practices. You approve the list.
Care that acts on it
PCP 1st enrolls each patient, supplies the blood pressure cuff and CGM, and its care managers call when a reading needs it. The patient keeps their primary care provider.
A report you can use
Every month, the ACO sees enrollment, reading adherence, escalations and open care gaps for the cohort, and which practices close flagged gaps.
