ACO360 is a program. PCP 1st and Caesar Health run it for the ACO. The ACO picks the patients, PCP 1st runs the care, and Caesar Health runs the operations. The first cohort is adults with type 2 diabetes and chronic kidney disease. Choose it when you want the care delivered for you.
Your own care programs run on your team. Your clinicians and care managers run remote patient monitoring, chronic care management, advanced primary care management and annual wellness visit outreach. Caesar's care management agents do the program work around them. Choose it when you have the team and want the work done faster.
You can do both: ACO360 for one cohort, your own programs for the rest of the panel.
What the care management agents do
- Find the patients. Program rules for RPM, CCM and APCM decide who qualifies. Patients due for an annual wellness visit are flagged on the pre-visit day sheet.
- Enroll them. Outreach calls and texts until each patient is reached, with every attempt logged.
- Watch the readings. Device readings come in, and out-of-range readings are flagged to your staff. Staff make every clinical call.
- Log the time and the work. Time and documentation are recorded for each patient, each month, so the review is ready when billing is.
Your clinicians make every clinical decision. Your practices bill for the programs they run.

Which EHRs does it work with?
Caesar works with athenahealth, ModMed and EZDERM through their APIs, and with other EHRs through its browser agent where there is no API. See EMR integrations.
