Patient communication platforms compared: Weave, Klara, Spruce, Luma, Artera and Caesar

A patient communication platform puts a practice's texts, calls and patient messages in one place, usually with reminders and digital forms on top. Weave, Klara, Spruce, Luma Health and Artera are the names practices compare most. They differ on three things: whether they run your phones, whether an AI answers, and which EMR they connect to.

This page lists six products in alphabetical order, including Caesar Health. We make Caesar, so check our row on a demo like every other. Every fact about a vendor comes from that vendor's own website, read on 2 November 2026.

The short version

  • Spruce and Weave sell a phone system. Both publish prices. Weave adds payments and reviews. Spruce adds fax, video and secure messaging, with a BAA in every plan.
  • Klara is part of ModMed. ModMed calls it "ModMed® Patient Engagement, powered by Klara®". It turns calls into text threads and uses AI to sort messages.
  • Luma and Artera sell AI agents. Both name many EHRs. Neither publishes a price.
  • Caesar sells agents that finish the task in your EMR. Calls, texts, intake and fax. It answers forwarded calls, so your phone number stays where it is.

Weave vs Spruce

Both run your phones and your texts. Both publish a price.

Spruce prices per user. $24 or $49 a month per user, as published on 2 November 2026, with a BAA included in every plan and trial. It adds e-fax, video visits and secure messaging. It names athenahealth and DrChrono integrations that sync contacts.

Weave prices per plan. From $199 a month, as published on 2 November 2026, with higher plans by quote. It adds payments, reviews and an AI Receptionist. It names medical PMS writebacks and an EMA by ModMed integration.

Weave's automatic booking is labelled "For qualified Dental PMS only". Spruce's AI transcribes and summarizes. Spruce has no published claim of an AI that answers calls.

How to choose

Start with who owns the phones. If you want a new phone system with texting attached, Spruce and Weave sell that. If your phones work and you want help with the calls, look at the agents.

Then ask what happens after the message. Ask who completes the booking, the filing or the callback: your staff, the patient or the software. Decide which work you want gone.

Check your EMR. Klara lives inside ModMed. Spruce names athenahealth and DrChrono. Luma and Artera name many EHRs. Ask each to show the exact read and write in your system.

Choose Weave if you want phones, texting, payments and reviews on one invoice, with up to 15 phones on the Ultimate plan.

Choose Spruce if you want a per-user price, a BAA in every plan, and phone, text, fax and video in one app for a small team.

Choose Klara if you run ModMed and want patient messaging inside your EHR vendor's own platform.

Choose Luma if you want action on open orders and referrals from faxes and your EHR, plus scheduling through AI conversations, from one vendor that names seven major EHRs.

Choose Artera if you want AI agents across calls, texts and notifications, connected through API, FHIR or HL7v2.

Choose Caesar Health if you want the agent that answers the call or text to book, reschedule or cancel in your EMR, and you want intake and fax handled in the same place. Your EMR stays. Your number stays.

You can run two of these. You can keep your phone system and add an agent. If you do, decide which product owns each reminder so patients don't get two.

What to ask on any demo

  1. Show me a reschedule by text land in my EMR. Live, in my system.
  2. Who answers a call at 7pm? An AI, a voicemail, or a call-to-text message?
  3. Which features need a specific EMR or specialty? Ask what's labelled dental-only or early access.
  4. What does it cost all in? Per user or per plan, setup, SMS registration, hardware, add-ons.
  5. Do I keep my number? Forwarding or a port? What happens to it if I leave? Read your current vendor's porting terms before you cancel.
  6. Will you sign our BAA? Before the first patient message.

Frequently asked questions

What is the best Weave alternative for a medical practice?

It depends on what you use Weave for. For phones and texting with a published price, Spruce. For patient messaging inside ModMed, Klara. For AI agents across many EHRs, Luma or Artera. For an agent that books in your EMR and leaves your phone number where it is, Caesar Health. See also the Weave alternative for medical practices.

What is a good Klara alternative?

Klara is ModMed Patient Engagement, powered by Klara. If you run ModMed and want to keep messaging in ModMed's platform, Klara fits. If you want texting plus a phone system, Spruce and Weave. If you want an agent that answers calls and writes the result into ModMed, Caesar has a live ModMed integration. See Klara, Weave and Caesar for ModMed practices.

What is a good Spruce Health alternative?

Weave if you want payments and reviews alongside phones and texting. Luma or Artera if you want AI agents with named EHR integrations. Caesar if you want the call or text to end in a booking in your EMR.

Who owns Klara?

ModMed. ModMed's own page calls the product "ModMed® Patient Engagement, powered by Klara®". On 2 November 2026, klara.com redirected to ModMed's patient engagement page.

Weave vs Spruce: which costs less?

It depends on headcount. As published on 2 November 2026, Spruce is $24 or $49 per user per month and Weave plans start from $199 a month. Spruce's price grows with each user; Weave's is per plan. Price both for your team.

Do Weave and Spruce have an AI receptionist?

Weave sells an AI Receptionist. Its automatic booking is labelled "For qualified Dental PMS only". Spruce publishes AI transcription and summarization of voicemails and call recordings. No published claim of an AI that answers calls.

Can I switch platforms and keep my phone number?

Usually, by porting it. Spruce says you can "transfer (port) your existing business phone or fax number". Caesar doesn't need a port at all: you forward calls from your current line. Check your current vendor's terms before you cancel.

Is Caesar Health a patient communication platform?

It covers the same channels: calls, texts and intake forms. The difference is the agent finishes the request in your EMR. Caesar isn't an EMR and doesn't replace yours.
